The forms available on this page are provided to help school districts comply with both Part B and Part C of the Individuals with Disabilities Education Act (IDEA). The forms can also assist districts in monitoring and documenting compliance, completing Corrective Action Plans (CAP) and other needs as necessary to comply with state and federal laws.

     The Corrective Action Plan (CAP) form is used by school districts to develop a plan to address systemic non-compliance identified in the final monitoring report. A CAP delineates action needed; person(s) responsible; timelines, and evidence of completion.

     Non-state personnel who complete work, such as peer monitoring, for the Minnesota Department of Education and seek reimbursement must use this form.
