The Department of Education receives and investigates reports of alleged physical abuse, sexual abuse, and neglect of public school students by school employees pursuant to *Minnesota Statute Section 626.556.

Reporting Student Maltreatment by School Employees- The confidential Reporting Form, posted below, should be completed and faxed or forwarded to the Student Maltreatment Program at MDE.
