Annual Reports & Plans

School districts are required to submit program and participant data and program plans to the Minnesota Department of Education (MDE). This page contains forms or links useful to program coordinators for preparing data, applications or plans for Early Childhood Family Education, Early Childhood Screening and School Readiness programs, and Interagency Early Intervention Committees.

School district Early Childhood Family Education, Early Childhood Screening and School Readiness data are electronically submitted to MDE by August 15th through the Early Learning Services Data System. The Early Learning Services Data System will be available May 5, 2008 through the "Data Submissions" Web page under the MDE "Data" tab.
