The Minnesota Department of Education, Early Learning Services, provides current information for school districts and their local interagency partners to be used in the administration and delivery of services to eligible infants and toddlers under Part C of the federal Individuals with Disabilities Education Act (IDEA).

The Family Outcomes Survey (Part C Version) is provided to families with children receiving early intervention services. The survey shall be given for all children who have been served by Part C for 6 months or more as the child turns age 3 or is exiting Part C for any reason. This family survey is available in Arabic, Cambodian, Croatian, English, Hmong, Laotian, Simplified Chinese, Oromo, Russian, Somali, Spanish and Vietnamese.

     Data will be collected utilizing the National Early Childhood Outcome (ECO) Center's Family Outcome Survey. The survey shall be given for all children who have been served by Part C and who have been in programming for 6 months or more as the child turns age 3 or is exiting Part C for any reason.
