How to Apply as a Sponsor of the Child and Adult Care Food Program (CACFP):

* CLiCS Sponsoring Authority Customer ID Request Form
* CLiCS Site Customer ID Request Form – submit one form for each “new” site
* CLiCS User ID/Password Request Form – submit form to request additional user IDs and passwords
* Listserv Subscribe/Unsubscribe Form – submit this form to receive important FNS electronic mail messages

Sponsors of more than one center need to submit CACFP Multi-Centers Management Plan and Budget information.

Application Renewal Using the Cyber-Linked Interactive Child Nutrition System (CLiCS):
Participating Child and Adult Food Care Program (CACFP) Centers must show their interest to continue with the program by following the Annual Renewal Instructions for Child and Adult Care Food Program (CACFP) Centers. Center sponsors that qualify for annual renewal are those that have an approved application in CLiCS with the Minnesota Department of Education - Food and Nutrition Service and have NOT been Nationally Disqualified.
