This section provides documents and resources used by School Food Authorities and State Agency staff in the process of completing USDA required monitoring and reviews. Examples of documents included are On-Site Review form, After-School Snack On-Site Review form and forms used by State Agency staff during Administrative Reviews.

Sponsor Monitoring


School Food Authorities with more than one site are required to complete an annual on-site review of all sites. The purpose of the review is to assist sponsors in assessing the meal counting and claiming system at each site. The review must be completed during a meal service and prior to February 1 of each school year. A follow-up review must be completed within 45 days when corrective action is needed.

After School Snack Program:  School Food Authorities that participate in the After School Snack Program are required to complete an on-site review for all sites twice during the school year. The purpose of the review is to assist sponsors in assessing compliance with program requirements. The review must be completed during a snack service time. The first review must be completed during the initial four weeks of operation; the second as determined by the sponsor.

State Agency Monitoring


Administrative Reviews:  The MN Department of Education is the State Agency responsible for conducting Administrative Reviews of all school food authorities participating in the National School Lunch Program. Administrative Reviews, also known as the CRE or Coordinated Review Effort, are completed by Program Specialists from the Food and Nutrition Service. Schools are reviewed at least once during a five-year cycle; not to exceed six years.

School Meals Initiative (SMI) Reviews:  The USDA requires that all school lunches be consistent with the recommendations of the Dietary Guidelines for Americans. Menus are monitored to ensure that specific minimum standards for calories and key nutrients are met. A nutrient analysis will be conducted on one week’s worth of menus. The State Agency will monitor menus for compliance with these nutritional standards at a minimum of once every five years. An SMI review may be conducted at the same time that an Administrative Review is scheduled or done as a separate program review.
