State Agency and Sponsor Monitoring-SFSP

The Minnesota Department of Education (MDE), Food and Nutrition Service (FNS) require SFSP sponsors to monitor program operations at sponsored sites. Below are sample monitoring forms and information about monitoring requirements. FNS also communicates changes, as necessary, for sponsor monitoring requirements.
