Evaluation Summary Reports

The Evaluation Summary Reports in this section are linked to the eight chronic or acute health concerns often associated with the Other Health Disability category. Each report is intended to provide sample documentation for teams as they determine eligibility and link needs, goals, and objectives to presenting problems as a result of the chronic or acute health condition. They are intended to be samples only and local district requirements must be considered when writing an evaluation summary report for an individual student.

These Evaluation Summary Reports were written in collaboration with the Other Health Disabilities Specialist and the Regional Other Health Disabilities Teams.

Please note: A medical diagnosis alone is insufficient to determine eligibility for special education services. Not all medical diagnoses are chronic or acute health conditions that impact educational performance. Some of these diagnoses may be considered in other special education disability categories depending on the presenting problems.
