Assurance of Compliance
The Assurance of Compliance form ensures that school districts do not discriminate in their use of funds provided through the Minnesota Department of Education (MDE). All school districts must complete the "Assurance of Compliance with State and Federal Law Prohibiting Discrimination" process annually by November 15.
There are two parts to this process:
Part One: Complete the “Certificate of Assurance of Compliance” form found below. It must be downloaded, signed, and mailed to:
